Community Care Services Program (CCSP)
- Also called Elderly and Disabled Waiver
- For individuals that are elderly and disabled
- Services provided include; personal support services, assisted living services, home delivered meals, emergency response system
- May have a cost share if SSI is over $781/ month
Service Options Using Resources in Community Environments (SOURCE)
- Also referred to as Elderly and disabled waiver.
- For adults ages 65 and older or 65 and under if disabled
- Must have a chronic condition that has been present for at least 3 months and must meet nursing home level of care.
- Services provided include; personal support services, assisted living services, home delivered meals, emergency response system
- Must be eligible for SSI coverage not exceeding $781/ month
Independent Care Waiver Program (ICWP)
- Individuals are aged 21- 64 years
- For individuals with physical disabilities as determined by Social Security Administration
- Individuals have severe physical impairment and or traumatic brain injury and meet nursing home requirement
- Services provided include; personal support services, assisted living services, home delivered meals, emergency response system
- Individuals are admitted on a first-come first served basis
Comprehensive Support Waiver (COMP)
- Formerly Community Habilitation Support Services (CHSS)
- Serves individuals’ with intellectual and developments disabilities who choose to remain the community
- Individual should be diagnosed before the age 22 and the diagnosis should be indefinite with an IQ <70
- Individual should have 3 or more functional impairments: self-care, learning, mobility, self-direction, capacity for independent living ,
- Requiring 24 hour care
- Services Provided; adult PT/OT/ST, behavioral supports, community living supports, community access, prevocational services, respite, supported employment, specialized medical supplies, specialized medical equipment, respite
New Options Waiver (NOW)
- FORMERLY Mental Retardation Waiver Program (MRWP)
- Serves individuals with intellectual or developmental disability
- Individual should have 3 or more functional impairments: self-care, learning, mobility, self-direction, capacity for independent living ,
- Individual is diagnosed before age 22 with an IQ of <70
- Services Provided; adult PT/OT/ST, behavioral supports, community living supports, community access, prevocational services, respite, supported employment, specialized medical supplies, specialized medical equipment, respite
Community Access Group (CAG)
- Purple Heart Activity Center
- For individuals with the NOW and COMP Waivers
- Individual comes to the day center based on allotted budget
Georgia Pediatric Program Waiver (GAPP)
- Serves children under the age of 21 who are medically fragile requiring skilled nursing.
- Individual should be under a physician’s care
- Must meet the level of care criteria for a hospital or nursing facility
- Services provided include; In-Home skilled nursing services and specialized day care services
