- Serves individuals with intellectual or developmental disability
- Individual should have 3 or more functional impairments: self-care, learning, mobility, self-direction, capacity for independent living ,
- Individual is diagnosed before age 22 with an IQ of <70
- Services Provided; Community Living Supports (CLS), Community Access Individual (CAI), prevocational services, respite, Supported Employment Individual (SEI), Specialized Medical Supplies (SMS), Respite, Community Access Group (CAG)
- Formerly Community Habilitation Support Services (CHSS)
- Serves individuals’ with intellectual and developments disabilities who choose to remain the community
- Individual should be diagnosed before the age 22 and the diagnosis should be indefinite with an IQ <70
- Individual should have 3 or more functional impairments: self-care, learning, mobility, self-direction, capacity for independent living ,
- Requiring 24 hour care
- Services Provided; behavioral supports, Community Living Supports (CLS), Community Access Individual (CAI), prevocational services, respite, Supported Employment Individual (SEI), Specialized Medical Supplies (SMS), Respite, Community Access Group (CAG)
