GAPP

 

• Serves children under the age of 21 who are medically fragile requiring skilled nursing.
• Individual should be under a physician’s care
• Must meet the level of care criteria for a hospital or nursing facility
• Services provided include; In-Home skilled nursing services and specialized day care services
